Complete these forms and submit them through the student health portal under the “Document Upload" section.
Vaccine Religious Exemption (PDF) – If you require a religious exemption to any vaccines, download this form, complete it, and submit it through the portal.
Vaccine Medical Exemptions (PDF) – If you have a medical exemption that prevents you from receiving the vaccines required by the State of NC, this form is to be printed off and taken to your physician for completion and then uploaded to the portal. This form can't be completed by a family member.
