Use these forms when completing your paperwork for financial aid.
2024-2025 Forms
Required Forms
The CSS Profile is now available for 2024-2025, and the FAFSA should be available by approximately January 1, 2024. The FAFSA is required for U.S. students only and is due by February 15.
Periodically Requested Forms
- Federal Verification Worksheet (required only for selected U.S. students)
- Federal Verification Worksheet for Dependent Students (available via IDOC)
- Federal Verification Worksheet for Independent Students (available via IDOC)
- Non-Filing Certification (required if student/parent did not file a tax return)
- Student, 2022 tax year (available via IDOC)
- Parent, 2022 tax year (available via IDOC)
- Outside Resources Notification Form (available by June)
Other Available Forms
- Noncustodial Parent Waiver Petition (required only for domestic students unable to submit their noncustodial parent's information)
- Unusual Circumstances (Dependency Override) Appeal Form (for students who believe they should be considered independent)
- Federal Direct Loan Application for Student Borrowers (available by June)
- Parent PLUS Loan Application (click on "Parent" tab; complete PLUS application and, if not previously submitted, MPN; no further action needed - available by June)
- Cost of Attendance Adjustment Form (available by June)
- Financial Aid Appeal (available by mid-December)
