Financial Aid Forms
Use these forms when completing your paperwork for financial aid.
Families without access to their tax materials may request substitute documents through the Internal Revenue Service (IRS):
- Need a substitute 1040? Request a "tax return transcript."
- Need a substitute W-2 or 1099? Request a "wage and income transcript."
- Need a non-filer's statement? Request a "verification of non-filing letter."
Periodically Requested Forms
Federal Verification Worksheet (required for U.S. students)
- Federal Verification Worksheet for Dependent Student (submit via IDOC)
- Federal Verification Worksheet for Independent Student (submit via IDOC)
Non-Filing Certification (required if student/parent did not file a tax return)
- Student - 2023 Tax Year (submit via IDOC)
- Parent - 2023 Tax Year (submit via IDOC)
IRS Transcripts (required if student/parent is unable to locate their tax return and/or W2(s)) https://www.irs.gov/individuals/transcript-types-for-individuals-and-ways-to-order-them
- Tax Return Transcript (in lieu of tax return)
- Wage and Income Transcript (in lieu of W2(s))
Outside Resources Notification
- Outside Resources Notification Form (be sure to complete the correct year's form, as a new form must be submitted for each year in which an outside resource is received)
Other Available Forms
- Noncustodial Parent Waiver Petition (required only for domestic students unable to submit their noncustodial parent's information)
- Unusual Circumstances (Dependency Override) Appeal Form (for students who believe they should be considered independent)
- Federal Direct Loan Application for Student Borrowers
- Parent PLUS Loan Application (complete PLUS application and, if not previously submitted, MPN; no further action needed)
- Cost of Attendance Adjustment Form
- Financial Aid Appeal (appeal window for 2025-2026 will open in mid-December)
